Canadian Academy of Osteopathy Launches 'Think First' Healthcare Pledge to Combat Reactionary Care

The Canadian Academy of Osteopathy (CAO) has announced the launch of a new personal pledge aimed at addressing a growing issue in modern healthcare: the overreliance on rigid protocols at the expense of critical thinking, reflection, and whole-person care.

The "Think First" Healthcare Pledge is a public commitment grounded in the academy's long-standing values and recent insights shared in its featured interview. The pledge encourages practitioners, students, and individuals to slow down, ask better questions, and take responsibility for how health decisions are made and experienced.

"Too much of healthcare has become reactionary," a senior CAO faculty member said in the interview. "When we stop thinking and start following instructions blindly, we miss the real problem." Another instructor noted, "Listening saves time. You find the real issue faster when you actually hear what a patient is telling you." These reflections shaped the pledge, which focuses on everyday behaviours rather than institutional promises.

Why this matters now: The World Health Organization reports that 74% of global deaths are linked to chronic disease, many tied to preventable factors. In Canada, over 50% of adults live with at least one chronic condition, often requiring long-term care. A national survey found that 57% of patients with chronic pain feel their needs are not fully met by standard systems. Healthcare burnout is rising, with more than half of providers reporting exhaustion.

The CAO's initiative directly challenges the current trend of protocol-driven care, which can overlook individual patient needs. By promoting critical thinking and active listening, the pledge aims to improve diagnostic accuracy and patient outcomes. The academy hopes that widespread adoption of the pledge will reduce misdiagnosis and unnecessary treatments, ultimately lowering healthcare costs.

The pledge is available on the CAO's website and is open to all healthcare professionals and the public. It includes specific commitments such as "I will listen before I act" and "I will question assumptions." The CAO encourages signatories to share their experiences using the hashtag #ThinkFirstHealth.
