Avident Health and Unite for HER Receive Pfizer and NCCN Grant to Improve Metastatic Breast Cancer Care

Avident Health LLC and Unite for HER have been awarded an independent quality improvement grant funded collaboratively by Pfizer Inc. and the National Comprehensive Cancer Network (NCCN). The grant supports a collaborative project that will deploy Avident Health's proprietary MyNavigator digital platform to enhance care delivery for patients diagnosed with metastatic breast cancer. This initiative matters because it directly addresses persistent gaps in care coordination between clinic visits, where patients and their caregivers often struggle to navigate complex treatment guidelines and access community-based supportive resources.

Living with metastatic breast cancer requires complex, ongoing treatment management in which adherence, symptom monitoring, and whole-person supportive care are critical. Yet patients and their loved ones frequently lack the tools to translate clinical guidelines into daily practice outside traditional appointments. The MyNavigator platform will serve as an interactive, home-based companion that delivers tailored education aligned with NCCN Guidelines for Patients, translating complex clinical pathways into accessible, bite-sized modules tailored to each patient's specific treatment journey. This empowers patients to participate actively in informed clinical decision-making.

The platform also streamlines access to integrative supportive care by directly integrating Unite for HER's comprehensive suite of free services—including professional nutrition counseling, fresh food delivery, mental health counseling, and evidence-based integrative therapies—into daily navigation prompts. Additionally, it uniquely engages informal family caregivers by including them in a secure, proactive communication loop, ensuring they have the context, education, and early symptom alerting needed to support treatment adherence at home. By capturing patient-reported check-ins and symptom trends, the platform enables proactive care team interventions that can prevent avoidable complications and reduce acute care utilization.

"Navigating metastatic breast cancer is a daily reality that extends far beyond the four walls of the clinic," said Dr. Maen Farha of Avident Health LLC. "By combining the proactive digital coordination of MyNavigator with Unite for HER's exceptional integrative services and NCCN's gold-standard guidelines, we are creating a continuous circle of support that empowers patients, supports clinical teams, and actively engages the family caregivers who are vital to treatment success."

Sue Weldon, Founder and CEO of Unite for HER, emphasized that collaborating on the MyNavigator platform enables the organization to reach patients and their families directly at the moment they need guidance, removing barriers to integrative nutrition, wellness, and emotional support resources. The project will measure key quality improvement endpoints over the grant period, evaluating improvements in patient knowledge concordance with NCCN Guidelines, rates of supportive service utilization, patient-reported decisional conflict, and therapy adherence metrics.

This grant-funded initiative holds significant implications for the metastatic breast cancer community. By bridging communication gaps and equipping both patients and caregivers with personalized education and proactive symptom tracking, the collaboration aims to foster shared decision-making, treatment adherence, and enhanced quality of life. For health systems and community practices, the platform promises to reduce administrative burden on clinical navigators while enabling earlier interventions. More broadly, the project signals a growing emphasis on integrating digital navigation with evidence-based guidelines and community supportive services to deliver continuous, patient-centered cancer care.
