The American Heart Association's first CarePlan Challenge awarded three digital health prototypes that integrate its science-based tools to transform cardiovascular care, highlighting the growing role of AI and personalized technology in evidence-based medicine.

American Heart Association Recognizes Three Health Tech Innovators in Inaugural CarePlan Challenge

The American Heart Association (AHA) has announced the winners of its inaugural CarePlan Challenge, an initiative designed to drive innovation in digital cardiovascular care. The challenge invited developers and health technology innovators to create prototypes integrating the AHA's science-based CarePlans—personalized care pathways for conditions such as heart failure, hypertension, and cardiac rehab—alongside tools like Life's Essential 8 and the PREVENT Risk Calculator. The three winning solutions, ConneQT, Porter Health, and OneVillage, were selected for their potential to transform preventive care, improve disease management, and enhance patient engagement.

As digital health technologies gain momentum, research indicates that patients are more receptive to AI-supported interventions when backed by clinical expertise and evidence-based guidelines. The AHA's Center for Health Technology & Innovation launched the CarePlan Challenge to harness this opportunity, providing applicants with API access to its digital tools. Submissions were judged on innovation, technical implementation, user experience, and clinical impact.

ConneQT offers guided wellness programs that link daily tasks, biometrics from the CONNEQT Pulse device, and personalized goals from the PREVENT calculator to build heart-healthy habits and provide clinicians with greater visibility into patient health. Porter Health is a web-based tool that delivers one-click PREVENT and cardiovascular-kidney-metabolic health risk assessments, along with an expert-vetted, AI-assisted CarePlan tailored to patient demographics. OneVillage focuses on women's heart health, combining physician visits, cardiac rehab, and supportive services into an 80-day personalized pathway.

“These innovators are pushing the boundaries to build a brighter future of cardiovascular care,” said Dr. Seth Martin, AHA volunteer and professor at Johns Hopkins Hospital. “Their prototypes aimed at using technology to turn science into action show significant promise in making evidence-based care more personal, accessible and impactful.”
